The UESP Interdisciplinary Team

The UESP Interdisciplinary Team of health care and social service providers works in partnership with the participant and caregiver, creating a comprehensive plan of care designed for each participant. It covers health care needs for as long as the participant remains enrolled in UESP.

The Interdisciplinary Team includes:

  • Primary care physician
  • Nurse practitioner
  • Day center manager
  • Home care nurse
  • Day center nurse
  • Nutritionist
  • Physical therapist
  • Occupational therapist
  • Recreational therapist
  • Social worker
  • Transportation staff
  • Nurse's aide

Services & Care for Participants

A majority of our services are provided at the PACE Centers in Dorchester and Roxbury. We also provide services at home and at our contracted facilities, including Boston Medical Center.

Services include:

  • Coordinated management of services
  • Interdisciplinary geriatric team
  • Individualized care plan for each participant
  • Comprehensive medical treatment and health care including
  • Primary & specialty care
  • Hospitalization
  • Rehabilitation
  • Prescription drug coverage
  • Therapies including nutrition, physical, occupational and speech
  • Day center program including activities, meals and snacks
  • In-home health and personal care
  • Transportation to the UESP PACE center and to medical appointments

Care Outside the Center

UESP will provide care as needed, in the participant's home or in one of our contract facilities. For hospital care, the participant is admitted to Boston Medical Center. For a necessary short or long-term stay in a skilled nursing facility, the UESP Program maintains contracts with nursing homes. The UESP team would continue to provide care as long as the participant is enrolled in UESP.

A participant must receive all needed health care, including primary care and specialist physician services (other than emergency services), from the PACE organization or from an entity authorized by the PACE organization.

PACE participants may be fully and personally liable for the costs of unauthorized or out-of-PACE program agreement services.
